The 1918 influenza epidemic's effects on sex differentials in mortality in the United States.

چکیده
THE 1918 INFLUENZA epidemic was a major demographic event in the United States and worldwide. It is notable for its virulence (over 20 million deaths worldwide, approximately half a million in the United States); its maleness (a difference between male and female age-standardized death rates of 174 per 100,000 ); and its W-shaped mortality age profile (death rates having a mode in the 25–34-year age group, strange for influenza, which usually has a U-shaped profile). This study presents a new finding from reexamination of published statistics on death: the 1918 influenza had a strong and fairly long-lasting effect on differential mortality by sex, diminishing the earlier female advantage. The mechanism we posit is a selection effect, whereby those with tuberculosis (TB) in 1918 were more likely than others to die of influenza. This outcome affected males more than females because TB morbidity was disproportionately male. The reduction of the pool of male TB cases lowered the male TB death rate in the years following 1918, and brought males’ life expectancy closer to the longer female life expectancy. Before going into detail about our reexamination, we briefly review some of the salient features of the 1918 influenza epidemic, which, in spite of its enormity, has not been a major focus of studies by demographers.

Influenza and Pneumonia Mortality in 66 Large Cities in the United States in Years Surrounding the 1918 Pandemic
The 1918 influenza pandemic was a major epidemiological event of the twentieth century resulting in at least twenty million deaths worldwide; however, despite its historical, epidemiological, and biological relevance, it remains poorly understood. Here we examine the relationship between annual pneumonia and influenza death rates in the pre-pandemic (1910-17) and pandemic (1918-20) periods and ...
متن کاملThe 1918 influenza pandemic hastened the decline of tuberculosis in the United States: an age, period, cohort analysis.
The effect of the 1918 influenza pandemic on other diseases is a neglected topic in historical epidemiology. This paper takes up the hypothesis that the influenza pandemic affected the long-term decline of tuberculosis through selective mortality, such that many people with tuberculosis were killed in 1918, depressing subsequent tuberculosis mortality and transmission. Regularly collected vital...
متن کاملNonpharmaceutical interventions implemented by US cities during the 1918-1919 influenza pandemic.
CONTEXT A critical question in pandemic influenza planning is the role nonpharmaceutical interventions might play in delaying the temporal effects of a pandemic, reducing the overall and peak attack rate, and reducing the number of cumulative deaths. Such measures could potentially provide valuable time for pandemic-strain vaccine and antiviral medication production and distribution. Optimally,...
متن کاملA Missed Summer Wave of the 1918–1919 Influenza Pandemic: Evidence From Household Surveys in the United States and Norway
Background. Reanalysis of influenza survey data from 1918 to 1919 was done to obtain new insights into the geographic and host factors responsible for the various waves. Methods. We analyzed the age- and sex-specific influenza morbidity, fatality, and mortality for the city of Baltimore and smaller towns and rural areas of Maryland and the city of Bergen (Norway), using survey data. The Maryl...
متن کاملA socially neutral disease? Individual social class, household wealth and mortality from Spanish influenza in two socially contrasting parishes in Kristiania 1918-19.
The Spanish influenza pandemic of 1918-19 was one of the most devastating diseases in history, killing perhaps as many as 50-100 million people worldwide. Much of the literature since 1918 has favored the view that mortality from Spanish influenza was class neutral. This view has prevailed, even though several contemporary surveys showed that there indeed were clear differences between the clas...
